Job Description
This role will be Monday - Friday 8AM - 5PM but some longer hours may be needed. After Orientation, there will be an opportunity to work remotely 1-2 days per week.
Experience
- 5 years of RN experience with 2 years of experience in critical care or emergency nursing with advanced clinical assessment skills
Preferred Qualifications
- Bachelor of Science in Nursing
- Previous experience in psychiatric or behavioral health
- Previous Case Management experience
- Experience in a medical inpatient/emergency department setting working with pediatric and adolescent patient populations
- Experience with behavioral mental health (BMH) facilities and placement procedures
- Previous Social Work experience a plus
Education
- Graduation from an accredited school of nursing
Certification Summary
- Licensure as a Registered Nurse in the single State of Georgia or Multi-State through the Enhanced Nurse Licensure Compact
- Certified Case Manager (CCM) or Psychiatric Mental Health (PMH-BC) Certification (current or within 2 years of hire)
- Basic Life Support (BLS) within 30 days of employment
Knowledge, Skills, and Abilities
- Ability to manage multiple and continually changing priorities and make rapid decisions
- Demonstrated ability in integrating pathophysiology, psychology, and sociology into the pediatric clinical setting
- Maintains continuity among nursing teams by documenting and communicating actions, irregularities, and continuing needs of patient care services.
- Effective verbal and written communication skill and ability to handle conflict and reach consensus and collaboration
- Must have computer experience and proficiency in a wide variety of software, e.g.., Microsoft Office applications and database management
Job Responsibilities
- Performs the nursing process while providing indirect patient care in a safe and effective manner that promotes quality patient outcomes. Functions as member of interdisciplinary healthcare team.
- Performs an initial discharge planning evaluation or Emergency Department assessment for patients.
- Provides supervision of Licensed Practice Nurse Navigator activities
- Cultivates and maintains effective interaction/communication with medical staff, nursing staff, social workers, and others to drive the care coordination process and facilitate continuity of patient care.
- Communicates with home health agencies, third-party payor, and other community resources as needed to coordinate needs.
- Facilitates clinical practice guidelines at the patient level.
- Performs outpatient and clinic care coordination and monitors patients' care amongst the hospital, clinic, and home.
- Facilitates care via telehealth to assist with patient needs and concerns.
- Provides health promotion activities and education for caregivers and staff at assigned facility, including workshops and trainings.
- Refers cases identified as risk management issues, peer review issues, or quality issues to appropriate personnel.
- Coordinates patient placement activities to optimize patient flow, patient outcomes, admission appropriateness, and cost efficiencies in support of organizational strategic initiatives while working with campus house supervisors, physicians, and nursing staff.
- Communicates and collaborates with leaders and staff at external organizations/agencies regarding placement needs for whole childcare.
- Maintains assigned databases.
- Collaborates in collection of data for identification and resolution of flow issues.
- Collects, analyzes, and interprets outcome data systematically to identify problem areas and assess need for improvement and/or support changes in system-wide processes in relations with the Behavioral Mental Health strategy.
- Prepares periodic reports as directed.
- Participates actively and contributes to organizational initiatives that impact patient flow.
- Supports continuous performance and quality improvement.
- Communicates openly with appropriate leadership team to ensure knowledge of key decision factors.
- Serves as expert clinical resource for patient placement by ensuring smooth functioning of the patient flow process to reduce wait times and avoid unnecessary patient movements.
- Maintains positive working relationship and demonstrates excellent customer services skills in all interactions between amongst the interdisciplinary team, physicians, internal customers, and external customers. Serves as a single point of contact for streamlining communication flow for escalated communication needs.
- Serves as a support liaison to the Behavioral Mental Health team.
- Empowers, promotes, and develops team members by fostering an environment which promotes professional development and problem-solving, maintaining and enhancing knowledge of current issues and integrating ideas into practice.
- Assists with orientation and continuing education of peers.
