In this role, you will play a crucial part in supporting patients post-hospitalization, ensuring a seamless transition back to their daily lives. Your expertise will be embedded within the Atlanticare Primary Care Physican Group, where you will provide invaluable assistance to patients, helping them navigate their care journey.
