MAJOR FUNCTION:
The Clinical Coordinator RN will operate in direct partnership with IMG practices. To assist our patients in navigating the various complexities of our health system and act as a resource to coordinate the patients' needs with our organization resources. This role will act as a clinical support for the IMG practices by support their pool's and provide clinical triage. This role will complete triage transfers by utilize protocols and clinical judgement to assess and prioritize a patients need and determine the appropriate course of action.
QUALIFICATIONS:
Education & Experience:
- Associate degree required.
- Bachelor’s degree preferred.
- Experience in acute inpatient, rehabilitation, sub-acute, skilled facility, homecare.
- Experience in Women's Health preferred.
- Experience in a care management role preferred.
- Experience in Ambulatory care preferred.
- 3-5 years of direct patient care experience required.
Certification/Licensure:
- Active New Jersey Registered Nurse License is required.
- Active Basic Life Support (BLS) certification is required.
