Position Responsibilities
As a Physical Therapist, you will help residents improve their strength, mobility, function, and independence through individualized, evidence-based care.
Responsibilities include:
- Perform comprehensive physical therapy evaluations and develop individualized treatment plans.
- Provide skilled physical therapy services in accordance with the California State Practice Act.
- Monitor resident progress and modify treatment plans as clinically appropriate.
- Supervise Physical Therapist Assistants (PTAs) and collaborate with Restorative Nursing staff to promote continuity of care.
- Complete accurate and timely clinical documentation, including treatment notes, progress reports, and recertifications.
- Collaborate with nursing and the interdisciplinary care team to support comprehensive resident care.
- Participate in care planning meetings and utilization reviews as needed.
- Educate residents and families regarding mobility, safety, home exercise programs, and discharge planning.
- Assess and coordinate appropriate durable medical equipment (DME), including walkers, wheelchairs, braces, and other mobility equipment.
- Promote safe mobility and functional independence throughout the rehabilitation process.
Qualifications
- Current and unrestricted California Physical Therapist (PT) license required.
