Pacific Coast Post-Acute in Salinas, CA, is seeking a motivated and passionate Outpatient Physical Therapist (PT) to join our growing rehabilitation team. Key Responsibilities

Perform comprehensive outpatient physical therapy evaluations to assess mobility, strength, balance, coordination, and functional limitations.
Develop and implement individualized treatment plans based on patient needs, goals, and evidence-based clinical practices.
Provide therapeutic interventions, including therapeutic exercise, manual therapy, neuromuscular reeducation, and functional mobility training, as appropriate.
Treat patients with orthopedic, neurological, and other conditions affecting daily function and quality of life.
Monitor patient progress, adjust treatment plans, and communicate outcomes with patients and the care team.
Maintain accurate, timely, and compliant clinical documentation in accordance with regulatory, payer, and facility requirements.
Collaborate with the Director of Rehabilitation and interdisciplinary team to support program growth and continuity of care.
Promote patient safety, engagement, and independence throughout the rehabilitation process.

Qualifications

Current, active California Physical Therapist (PT) license.
Doctor of Physical Therapy (DPT) or equivalent qualifying degree.
Outpatient orthopedic, sports medicine, or neurological rehabilitation experience preferred.
New graduates with strong outpatient clinical rotations are encouraged to apply.
Excellent communication, organizational, and clinical problem-solving skills.
A commitment to evidence-based practice, patient-centered care, and positive rehabilitation outcomes.
Interest in contributing to the development and growth of an outpatient rehabilitation program.
