- Connect & Collaborate: Forge powerful connections across internal departments, with vital referral sources, and with our physician partners, ensuring seamless coordination and a cohesive patient experience.
- Drive Excellence: Champion exceptional service and meticulous follow-through from the initial referral to the final admission, making a tangible difference in patient lives daily.
Qualifications:
- An active and unrestricted New York State Registered Nurse (RN) license.
- Proven, hands-on intake experience within a Certified Home Health Agency (CHHA) or Managed Long-Term Care (MLTC) setting – you know the landscape!
- A demonstrated track record of visionary leadership, impeccable organizational prowess, and exceptional communication skills that inspire confidence and collaboration.
