As a Pharmacy Clinical Specialist – Heart Failure, a caregiver in this role will provide advanced clinical pharmacy services within Cleveland Clinic’s Ambulatory Heart Failure Clinics, serving as a key pharmacy resource for complex cardiovascular and heart failure patients. Practicing under a collaborative practice agreement, the caregiver will independently initiate and adjust heart failure and complementary medication therapies, including guideline-directed medical therapy (GDMT) initiation and titration and GLP-1 therapy titration. The caregiver will also support pre- and post-cardiac transplant patients and collaborate closely with cardiologists, transplant teams and interdisciplinary clinic staff to optimize medication therapy and patient outcomes. While primarily focused on heart failure, this role will maintain flexibility to support other acute care areas across the Heart, Vascular & Thoracic Institute as operational needs require.
