Position Summary 💊 Clinical Pharmacist – Transitions of Care 🩺 Join Orlando Health and make an impact on patient care! We’re seeking a Clinical Pharmacist – Transitions of Care to provide safe, effective pharmaceutical care while collaborating with healthcare professionals to support our patients. ✨ What You’ll Do 💊 Dispense medications and IV fluids according to established standards and procedures 🩺 Develop and monitor pharmaceutical care plans 📋 Provide pharmacotherapy and pharmacokinetic consultations, as applicable 💻 Utilize pharmacy and healthcare computer systems to support patient care 🔍 Evaluate medication-related alerts, including IV-to-PO and renal dosing protocols ⚕️ Identify, investigate, and document adverse drug reactions and medication errors 🤝 Participate in multidisciplinary rounds, when applicable ❤️ Provide excellent service and communication to patients, families, physicians, visitors, and team members 🎓 Education Bachelor of Science in Pharmacy or PharmD required 📜 Licensure Current Florida Board of Pharmacy license required 🏥 Experience Hospital pharmacy experience preferred 🌟 What We’re Looking For A pharmacist who is organized, patient-focused, collaborative, and committed to providing high-quality pharmaceutical care while continuing to grow within a health-system pharmacy environment. 💙 Ready for your next opportunity in pharmacy?
