About the Role
We are seeking an experienced Clinical Pharmacist to join our award-winning Wellness Support Team on a 12-month fixed term basis to provide maternity leave cover.
This is an opportunity to step beyond a traditional pharmacy role and become an integral part of a multidisciplinary healthcare team. Rather than working solely behind the scenes, you will work directly with patients, whānau, General Practitioners, Nurses, Health Improvement Practitioners, Health Coaches, and other healthcare professionals to improve health outcomes through safe and effective medicines management.
Working across Counties Manukau and other regions, you will help patients better understand their medicines, overcome barriers to treatment, and gain the confidence to manage their health. Through meaningful clinical interactions and collaborative care planning, you will play an important role in improving access to healthcare, supporting people with long-term conditions, and reducing health inequities within our communities.
This role offers variety, autonomy, and the opportunity to work at the top of your scope while building strong relationships with both patients and clinical teams. You will have the opportunity to influence treatment decisions, support service development, and contribute to innovative models of care that place patients and whānau at the centre of everything we do.
What You Will Do
As a valued member of the clinical team, you will:
- Undertake comprehensive medication reviews and clinical assessments.
- Optimise medication use to improve patient safety and health outcomes.
- Support medication reconciliation and safe prescribing practices.
- Provide medicines education and support to patients and their families.
- Improve medication adherence and patient understanding of treatment plans.
- Support the management of long-term conditions through evidence-based clinical recommendations.
- Work collaboratively with General Practitioners, Nurses, and multidisciplinary teams to deliver coordinated care.
- Contribute to quality improvement initiatives, medication audits, and service development activities.
- Promote culturally safe practice and equitable health outcomes.
- Support healthcare teams with clinical advice and medicines management expertise.
What You Will Bring
To be successful in this role, you will have:
- A Bachelor of Pharmacy or equivalent recognised pharmacy qualification.
- A current Annual Practising Certificate without restrictions.
- A postgraduate clinical pharmacy qualification, or a commitment to working towards one.
- At least two years of recent clinical experience within primary care or hospital pharmacy.
- Excellent communication, relationship-building, and critical thinking skills.
- A strong understanding of Te Tiriti o Waitangi and its application within healthcare delivery.
- Knowledge of health inequities relating to medication access and use in Aotearoa New Zealand.
- The ability to build strong relationships with patients, whānau, and healthcare professionals.
- A commitment to culturally safe, patient-centred care.
The following would be highly advantageous:
- Experience working within multidisciplinary healthcare teams.
- Experience providing clinical advice and support to healthcare professionals.
- Experience working with diverse Māori, Pacific, and multicultural communities.
